Myomas, or Uterine Fibroids, are benign tumors occurring in over 1/3 of women. They can be found within the uterine muscle (intramural myomas), on the external surface of the uterus (pedunculated or subserosal myomas), or in the uterine lining (submucosal myomas). Some women are asymptomatic while others experience heavy bleeding, pain, bloating, urinary frequency, or infertility.
The da Vinci robot enables the removal of fibroids using the endowrist instrumentation. The robotic wristed instrument can rotate nearly 360 degrees, allowing seven degrees of motion and flexibility far superior to conventional laparoscopy.
In addition, it facilitates access to fibroids in difficult anatomic locations. Because the da Vinci robot allows Dr. Orbuch to visualize the fibroid and uterus in a magnified 3-D image, small blood vessels can be seen and sealed in order to minimize blood loss.
The da Vinci robotic instrumentation is miniaturized, which allows for very delicate manipulation of tissue. Motion scaling software also reduces a surgeon’s actual hand movements to much smoother and finer instrument movements.
The da Vinci robotic myomectomy is providing women an opportunity to retain their uterus and subsequently, retain their fertility. Laparoscopic myomectomies often take longer than open abdominal myomectomies, and many are converted during surgery to an open abdominal incision if the uterus fibroids are too large and/or there are many fibroids.
Robotic myomectomies are performed on an outpatient basis and women can return to work in approximately one week.
